🩺New Patient or Established Patient Annual Visit🩺

$145.00

New Patient Annual Visit

This visit is both a preventive exam and an initial intake for the clinic. It takes longer because the provider must build your medical profile from scratch. Typically A full review of your medical, surgical, family, and social history. A detailed review of medications, allergies, and past treatments. A comprehensive physical exam. Preventive screenings based on age and risk factors. Discussion of lifestyle, mental health, and long‑term health goals. Establishing your chart, baseline vitals, and future care plan.

Established Patient Annual Visit

This visit focuses on ongoing preventive care because the clinic already knows your history. What typically happens

Updates to your medical history since your last visit. Review of any new symptoms, medications, or diagnoses. Routine physical exam, Age‑appropriate preventive screenings. Review of chronic conditions and long‑term health goals. Adjustments to your care plan if needed

🩺Established Patient Urgent Visit🩺

$100.00

At Tafoya Family Practice, your health is our priority.

Our urgent visits provide prompt, compassionate treatment with same‑day and next‑day appointments available to help you get care when you need it most. A same‑day urgent visit at a primary‑care clinic is a quick, problem‑focused appointment your regular doctor’s office offers when you need care today, but the issue isn’t severe enough for the ER. It’s meant to bridge the gap between routine appointments and urgent care centers by giving you fast access to someone who already knows your medical history.

🩺Established Patient Follow-Up Visit🩺

$75.00

Continuity of care improves health outcomes

Medication review — The clinician asks how the medication has been working, whether you’ve had side effects, and whether anything in your health has changed.

Vitals or basic checks — Depending on the medication, they may check blood pressure, weight, or other simple measurements to ensure the medication is still appropriate. Chart and history update — They confirm your medical history, allergies, and any new diagnoses or medications from other providers. Refill decision — If everything looks safe, they send a new prescription to your pharmacy. Some medications require more frequent in‑person monitoring than others. Discussion of alternatives — If the medication isn’t working well or you’re having trouble accessing it, they may talk through other options.

🩺Influenza Vaccine🩺

$35.00

Getting a flu vaccine each year is one of the most effective ways to reduce your risk of serious illness.

Influenza can lead to complications such as pneumonia or hospitalization. Annual influenza vaccination is recommended, starting in September or October, because it takes about two weeks for your body to build protection. Even if you miss that window, it’s still worthwhile to get the shot later in the season, since flu activity can continue into spring. Flu vaccines are typically available from October until March.
